How To Register as a Victim
If you are the victim of a crime, or have participated in the criminal prosecution of a case, and you wish to receive information regarding an offender, you MUST notify the Victim Notification Program in the following manner:
- Obtain a Victim Bill of Rights Notification Request Form either from this web page (to access form, complete it, select legal paper size in page set-up, and print click here) or contact the prosecuting attorney's office IN THE COUNTY IN WHICH THE CRIME TOOK PLACE. Click here to access a list of county attorneys and their addresses. A member of the prosecutor's office staff can provide you with any help needed in completing the form.
- Be sure you complete the form with all information requested, including the personal information, to insure we will be able to contact you.
- Obtain the prosecutor's written certification (located on the form) so we know you are eligible to receive victim notification.
- After the form is completed, signed and certified by the prosecutor, mail the form to:
